	Health: I am in overall good health and meet medical guidelines.
	Commitment: I am willing to donate to any patient in need. I understand that while donation is purely voluntary, the registry is counting on me should I be called as a match someday. Withdrawal at that time would give false hopes to the patient(s) in urgent need.
	I live in the United States or Canada: If you live outside the United States or Canada, please contact a registry in the country where you live.
	I have not already joined: When you are tested to become a potential donor, you only need to do it once. Most worldwide registries are connected through one international registry. If you have previously donated a blood sample or cheek swab and have a donor ID card or receive mail from the registry, you do not need to join again. For more information, please call 800-9MARROW.
	Payment: I understand that unless I have a sponsorship code or am the recipient of an Online Donor Gift I will have to pay $54 to register. This covers the cost of my tissue typing test performed at the laboratory and the supplies ONLY. Gift of Life sponsors all other expenses, including shipping. If I have a sponsorship code, my test will be underwritten by a sponsor. I will also have the option to pay for my test to help "pay it forward!" More Information on the cost
